An R.N. Case manager will coordinate all aspects of care, evaluate the patient needs, implement the physician treatment plans and monitor the care and condition of the patient.

Certified Home Health Aides are available to assist with bathing, personal care, hygiene, ambulation and daily living activities.

Choice Home Health is a Participating Provider for Blue Cross® / Blue Shield®, Healthchoice, Medicare, Medicaid, United Healthcare, Humana, AARP, Global, CL Frates, ACS, Homelink, Sterling, Health Smart and Carecentrix. Call for additional plans.

Qualifications for Home Care are based on the payers’ rules. To receive this service as a Medicare benefit, clients must be homebound, need intermittent skilled services and must be under the care of a physician.
